1. Click the title of the outcome (hyperlinked in blue)
  2. If assessment is performed at the level you currently are in your organizational structure, click on the "Assessment" box.  If not, use the "Relate" feature.
  3. Complete the Assessment form and click "Submit".
  4. Add any assessment related attachments (prompts, sample work, etc.) by clicking on "Attachments" and dropping the files where indicated.
